Incidental Lung Nodule Detection Rates in Patients with Thyroid Cancer

ÖZET
Tiroit kanserleri tüm dünyada olduğu gibi ülkemizde de insidansı artan, en sık karşılaşılan endokrin kanserdir. Kadınlarda daha sıklıkla görülen tiroit kanserleri ülkemizde de kadın popülasyonu içerisinde en sık görülen 2. kanserdir. Hastaların küçük bir kısmında tanı koyulduğu sırada en sık akciğerler olmak üzere hematojen yayılım vardır. Genel olarak prognoz iyi olmasına rağmen uzak metastazı olan hastalarda mortalite ve morbidite artar. Tiroit kanserlerinin primer tedavisi total tiroidektomidir. Retrospektif olan çalışmamıza 2020 yılı içerisinde Sivas Cumhuriyet Üniversitesi Tıp Fakültesi Nükleer Tıp Anabilim Dalı’nda diferansiye tiroit kanseri nedeniyle yüksek doz radyoaktif iyot (RAI) tedavisi almış tüm hastalar dâhil edildi. Çalışmaya dâhil edilen 37 hastadan 25’i kadın, 12’si erkekti ve median yaş 49’du. Hastalardan 35’i papiller karsinom (%94.6) iken 2’sinde Hurthle hücreli karsinom (%5.4) vardı. Ablasyon başarısı ile akciğerde nodül varlığı arasında istatistiksel olarak anlamlı ilişki bulunmadı (p=0.959). Akciğerdeki nodül sayısı ile tiroit tümör odak sayısı ayrıca hastaların yaşı ve cinsiyeti ile de istatistiksel olarak anlamlı ilişki bulunmadı.

ABSTRACT
Thyroid cancers are the most common endocrine cancer with an increasing incidence in our country as well as all over the world. Thyroid cancers, which are more common in women, are the second most common cancer in the female population in our country. At the time of diagnosis, a small part of patients may have hematogenous spread, most often to the lungs. Although the prognosis is generally good in patients with thyroid cancer, the mortality and morbidity rate may increase in patients with distant metastases. Primary treatment of thyroid cancers is total thyroidectomy. Our retrospective study included all patients who had received high-dose radioactive iodine (RAI) treatment due to differential thyroid cancer in Sivas Cumhuriyet University Faculty of Medicine Department of Nuclear Medicine in 2020. A total of 37 patients were included in our study. 25 of the patients were female and 12 were male, and the median age of the patients was 49. While 35 of the patients (94.6%) had papillary carcinoma, 2 had Hurthle cell carcinoma (5.4%). No statistically significant correlation was found between the success of ablation and the presence of lung nodules (p = 0.959). No statistically significant relationship was found between the number of lung nodules and the thyroid tumor foci number, as well as the age and gender of the patients.
